1. It has been brought to my
attention that you have not completed the Electronic Personnel
Security Questionnaire (SF 86) as requested on 7 May 2003 by the ###th
Communication Flight
Security Manager.
2. The Standard Form 86 is required to obtain or maintain the current
level of clearance needed
for your career field as per Air Force Manual 36-2108, Enlisted Classification:
“For award and retention of
AFSCs 3C031/51/71, eligibility for a Top Secret security
clearance according to AFI
31-501, Personnel Security Management Program.”
3. You are hereby reprimanded. You have engaged in aberrant behavior which
will not be
tolerated from a member of the United States Air Force. Your past behavior has
raised serious
questions about your ability to remain in the United States Air Force. As a
member of the ###
Communication Flight, you have the responsibility to acquire the Security
Clearance required by
your Air Force Specialty Code (AFSC). You have been given ample opportunity to
supply the
Unit Security Manager with the paperwork needed for the submission of your
Security package.
4. Without your Top Secret Clearance you are unable to perform your 3C0
career duties which
include receiving, handling and processing Top Secret Information. Failure to
comply will result
in the immediate transfer or discharge.
5. PRIVACY ACT. AUTHORITY: 10 U.S.C. 8013. PURPOSE:
To obtain any comments
or documents you desire to submit (on a voluntary basis) for consideration
concerning this
action. ROUTINE USES: Provides you an opportunity to submit comments or
documents for
consideration. If provided, the comments and documents you submit become a part
of the action.
DISCLOSURE: Your written acknowledgment of receipt and signature is
mandatory. Any
other comment or document you provide is voluntary.
6. You will acknowledge receipt and return this letter to me within 3
workdays of your receipt.
Your signature on this document is solely for receipt purposes and is not an
admission of guilt.
Any comments or documents you wish to be considered concerning this LOR must be
included
with your response.
