SUBJECT: Letter of Reprimand - Late for Duty

1. On <DAY> <MONTH> <YEAR>, you were late for work. Which made you derelict in your duties and responsibilities, and this will not be accepted in the Air Force. Further investigation disclosed that this is your third such offense. You are a Non-Commissioned Officer who has a responsibility not only to yourself but to the Airmen you lead. They look up to you for guidance. You are supposed to be setting the example and you are failing.

2. Air Force Instruction (AFI) 36-2618, The Enlisted Force Structure, Chapter 4 - General NCO
Responsibilities states:

“Accept and execute all duties, instructions, responsibilities, and orders in a
timely manner.”

“Correct marginal or substandard behavior or duty performance.”

3. You are hereby counseled, for failure to obey order or regulation which is indirect violation of AFI 36-2618, Article 86, Absence Without Leave; Late for Duty, and Article 92. In the Air Force, failure to comply with Article 92 of the UCMJ will not be tolerated. I can assure you that many airmen have been given Article 15’s and have been court-martialed for similar conduct. This unit, this wing, and indeed the entire Air Force can not function with undependable members. In light of the current world situation, this is even more vital than ever. A nations’ military is only as good as its people. Our dependability is absolutely critical. Ignoring these rules and regulations will no longer be tolerated. I expect nothing more than a complete turnaround of your personal behavior/conduct on and off duty. You will conduct yourself in a manner above reproach. I expect you to always take care of your responsibilities by doing the right thing even in the absence of supervision, it is called integrity. Failure to do so will result in more severe administrative action.

4. PRIVACY ACT STATEMENT: AUTHORITY: 10 U.S.C. 8013. PURPOSE: To obtain any comments or documents you desire to submit (on a voluntary basis) for consideration concerning this action. ROUTINE USES: Provides you an opportunity to submit comments or documents for consideration. If provided the comments and documents you submit comments or documents for consideration. If provided, comments and documents you submit become a part of the action. DISCLOSURE: Your written acknowledgement of receipt and signature are mandatory. Any other comment or document you provide is voluntary.

5. You will acknowledge receipt and return (or send) this letter to me within 3 workdays of your receipt. You will also include any comments or documents you wish considered concerning this LOR with your response.
