MEMORANDUM FOR MADE A MISTAKE, AMN, FR123-45-6789

FROM: 4 XXX/CC

SUBJECT: Letter of Reprimand

1.  Investigation has revealed that on <DATE> at or near <TIME> hours you were consuming alcohol in <LOCATION>, with an Air Force member who was under the age of 21.  You knew or reasonably should have known the individual was not of legal age to consume alcohol, yet you witnessed him consume alcoholic beverages without attempting to deter or stop him from doing so.   You were subsequently apprehended, along with seven other unit members, by Security Forces when the underage drinker/minor in possession was identified in your immediate area.  You of all people should have known better considering you have been punished recently for your own indiscretions involving alcohol and underage drinking.

2.  You are hereby reprimanded.  Your actions are a violation of Article 92-Derelection of Duty, Uniform Code of Military Justice.  Allowing another military member to break the law without taking reasonable steps to stop them is a crime.  As an enlisted member, you took an oath to obey the orders of the Officers appointed over you, according to the regulations and the Uniform Code of Military Justice.  By violating this oath, you have caused me to question your professionalism and dedication.  At your in-processing briefing to the unit, a recent Commander’s Call, and during Wingman Day activities on <DATE> every member of this squadron was specifically tasked to look out for their fellow Airmen.  I am disappointed that you had the opportunity to keep someone out of trouble, but failed to act.  Airman <NAME>, your career in the Air Force cannot support any more alcohol related incidents.  In the future, I expect you to do a better job of looking out for your fellow airmen.  Rest assured than future misconduct of this type will result in more severe disciplinary action.  I fully intend to add this reprimand to your Unfavorable Information File (UIF).

3.  Privacy Act Statement:  AUTHORITY - 10 U.S. C8013.  PURPOSE - To obtain any comments or documents you desire to submit (on a voluntary basis) for consideration concerning this action.  ROUTINE USES - Provides you an opportunity to submit comments or documents for consideration.  If provided, the comments and documents you submit become a part of the action.  DISCLOSURE - Your written acknowledgment of receipt and signature, by endorsement below, are mandatory.  Any other comment or document you provide is voluntary.

4.  You will immediately acknowledge receipt and return this letter to me within three workdays of your receipt with any comments or documents you wish to be considered concerning this action.
