WHEN TO REFER A REPORT:

  •  If an evaluator marks “Does Not Meet Standards” in any performance factor, Section III, or a rating of “1” in Section V
  •  If comments made in the report are negative in nature.

WHO CAN REFER A REPORT?

  •  Rater or Additional Rater/Reviewer

WHEN REFERENCING ARTICLE 15 ACTIONS, CIVIL OR COURT-MARTIAL CONVICTIONS, PUNITIVE/ADMIN ACTIONS…

  •  Comments must identify the underlying conduct/behavior that led to the action:

    Ex: During this reporting period, TSgt Donuts sexually harassed a subordinate for which he/she received an Article 15

THE REFERRING EVALUATOR/RATER

  •  Prepares the referral memorandum after the close-out date of the report; meaning, the memorandum itself must be dated the after the close-out date of the report.
     
  • Hand delivers the memo and a copy of the report to the ratee and obtains ratee signature and date acknowledging receipt. (RATEE SIGNATURE ON MEMO ONLY, NOT THE REPORT)
  • Provides a copy of the memo to the ratee.
  • Inform the ratee that they have 10 calendar days to submit a rebuttal.
  •  DO NOT COMPLETE ADDITIONAL RATER/REVIER COMMENTS.

THE ADDITIONAL RATER/REVIEWER

  •  If the additional rater /reviewer decides to refer a report due to a rating or comment made by the previous evaluator (rater), they will refer it to the ratee BEFORE completing his/her portion of the report. The referral memo will instruct the ratee to direct/return any rebuttal comments to him/her.

RATEE

  •  Will acknowledge receipt of the referral memo by signing and dating it. NOTE: This signature DOES NOT signify concurrence with the report or indicate whether or not they will submit a rebuttal.
  •  Ratee has 10 calendar days to submit a rebuttal. See below for rebuttal guidance for ratee:
    •  May ask the MPS Force Management office for guidance in preparing rebuttal comments.
    •  May have another individual prepare comments on his/her behalf. If this is done, ratee must include statement confirming this document is to be considered as the ratee’s response. Can be on same document or separate document. All documents, separate or not are considered one of the 10 pages to which the rebuttal is restricted.
    •  May not make any comments that reflect on the character, conduct, integrity, or motives of an evaluator unless fully substantiated/documented.
    •  ALL pertinent attachments become part of the report filed in the personnel record.
    •  10 PAGES IS THE MAXIMUM ALLOWED; A NECESSARY RESTRICTION DUE TO SPACE LIMITATION IN PERSONNEL RECORDS AND SELECTION FOLDERS.

Upon receipt of rebuttal or when 10 calendar days have elapsed the evaluator completes his/her portion of the report.
If rebuttal is submitted, the evaluator named in the referral memo will consider ratee’s comments (as should any subsequent evaluator). The rater/evaluator will then prepare an endorsement to the report and will enter the appropriate statement IAW AFI 36-2406, Chap 3.9.7

ALL FINAL SIGNATURES ON REFERRAL REPORT MUST BE “WET” (INK SIGNATURE) AND REPORT MUST BE PRINTED HEAD TO FOOT.

**UPON COMPLETION OF REPORT, SUBMIT ALL ORIGINAL DOCUMENTS TO THE FORCE SUPPORT SQUADRON  EPR/OPR OFFICE OR YOUR CSS**
