  • PCS/PCA Decorations will be based on EPRs/OPRs
  • Achievement Decorations read like awards, and will need a narrative for justification
  • DO NOT plan on making up mid-tour points in a PCS medal
  • Ask for HELP, the only person looking out for #1 is you
  • The only person looking out for your Airman is YOU!
  • ASK for/WRITE a Mid Tour decoration 
  • WRITE!!! The more you do it, the better you get
  • Use F7 to spell check. Errors reflect badly on you and your subordinate!

MANDATORY OPENING SENTENCE:
Major John M. Doe distinguished himself by (heroism) (extraordinary achievement) while participating in aerial flight as (duty assignment) (at or near) (on_____) OR (from_____ to ____).

NARRATIVE:

Heroism: On that date while piloting F-15 aircraft Major Doe’s aircraft suddenly exploded and became engulfed in fire over a heavily populated city. Despite the hazardous condition encountered, Major Doe successfully guided his aircraft away from the populated area before ejecting himself from the disabled aircraft.

Extraordinary Achievement: During this period, Major Doe’s exemplary knowledge and outstanding airmanship, displayed under extremely hazardous conditions, culminated in highly successful aerial penetrations into Hurricane_______. The accuracy of data obtained during these penetrations provided essential early warning information on location, wind velocity, and direction of one of the most destructive hurricanes to hit the United States mainland.

CLOSING SENTENCE:
The outstanding heroism and selfless devotion to duty displayed by Major Doe reflect great credit upon himself and the United States Air Force.
OR
The professional competence, aerial skill, and devotion to duty displayed by Major Doe reflect great credit upon himself and the United States Air Force.

For Posthumous award: The outstanding heroism and selfless devotion to duty displayed by Major Doe in the dedication of his service to his country reflect great credit upon himself and the United States Air Force.

NOTE 1: No more than 10 lines when using 12 Pt Font for the DFC

NOTE 2: No more than 12 lines when using 10 Pt Font for the DFC

NOTE 3: Must use opening/closing sentences as indicated in this template. Must leave 3 inch margin at the bottom for official seal. Orientation for the citation is landscape
