The following Air Force Medical Service Award, Surgical Excellence Award of the Year, example has been taken out of an IMT document.  I have changed mission information to protect myself and the people I have received the information from.  I will attempt to give a little insight to the entire process if I know it.

Comments:

The biggest problem that readers cannot see on this is the fact in the 1206 title was spelled: Sugical Excellence Award.  Notice Surgical?  Although this was a problem, the duty title was 45E3/Chief, Ophthalmlogy Element, where as Ophthalmlogy ought to be Ophthalmology.  Finally, there were no acronyms listed in the 1206.
